PORT ANGELES – The Home Health Quality Improvement (HHQI) campaign named Olympic Medical Home Health as quality improvement agency of the month in January.
Thousands of home health agencies and other providers across the country participate in the voluntary HHQI initiative, a campaign that seeks to reduce avoidable hospitalizations and improve the management or oral medications for home health patients.
Olympic Medical Home Health received specific recognition in January.
“When we serve our community, we know we are taking care of our friends and neighbors. We take this responsibility to care for them very seriously, and partner with our patients to help them successfully remain independent in their homes,” says Fran Sisson, administrator, Olympic Medical Home Health. “We take pride in our chronic care management program and care transitions. Our quality clinical care has kept our overall unnecessary hospitalization rate below the national average, and our 30-day re-hospitalization rate low as well.”
The HHQI campaign currently has more than 10,000 participants from 5,500 agencies across the nation.
