Functions in an assistive role, under the supervision of a Registered Nurse or Therapist, providing personal care services as directed to meet the patient’s needs. 



High school diploma or GED.  Completion of State approved Certified Nursing Assistant Program.



Minimum of one year experience in long term or home health care preferred.



·        Possess and maintain Basic Life Support (BLS) certification.

·        Current/valid Washington driver’s license.

·        Driving record in compliance with OMC standards currently in effect or as may be amended.  See HR Policy 3.12 Driving for OMC Business.

·        Current automobile insurance.



·        Must have a cell phone (portion of cost reimbursed by OMHH).

·        Must have computer internet connectivity (OMHH work laptop provided).

·        Reliable automobile transportation to travel to patient residences.
