JOB SUMMARY

The Social Work Care Manager I intervenes with patients who have complex psychosocial needs, who require assistance with eligibility determination for social programs and funding sources, and who may qualify for community assistance from a variety of  resources and agencies. In addition, this position offers crisis intervention and/or other appropriate interventions to patients and families with psychosocial needs, and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. This role will coordinate a shared caseload with the Nurse Care Manager and/or receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, case managers, staff nurses from OB, SSU, ER) as assigned.

QUALIFICATIONS AND EQUIPMENT COMPETENCIES

EDUCATION

·        Master’s of Social Work preferred

·        Equivalent or related degree may be considered

 

EXPERIENCE

·        One to two years’ social work experience in social work setting, protective services, or other social service provision

·        Experience in medical setting preferred

 

LICENSURE/CREDENTIALS

·        Basic Life Support (BLS) certification

·        SBIRT certification within six months of hire
